3. Mordekaiser
With the new AP item adjustments, Mordekaiser benefits in a big way. He is a simple, but also extremely effective champ. Currently, he got a win rate of 52.3%.
Build:
The core items for Mordekaiser will pretty much always be Rylai's Crystal Cepter into Riftmaker. Afterward, you can go for Liandry's Torment and Jack'Sho, the Protean. Other great item options are Thornmail or Zhonya's Hourglass.
Powerspikes:
Mordekaiser got lots of small powerspikes, including level 3, level 6 and level 9 with his Q maxed out for example. Generally speaking, he got a great laning phase and therefore is an early and mid game champ.
Counters:
Mordekaiser struggles against Kayle. When she gets ulted by Morde, she just uses her own ult and is protected. Other counters are Gwen and Vayne. A good Gwen can deny your R with her W. Vayne on the other hand just outranges you and gives you a hard time landing your abilities with her Q.
2. Vayne
With huge improvements on Terminus this patch combined with the buffs on Guinsoo's Rageblade last patch, Vayne will destroy any opponent in the top lane being even more annoying than she already was. Currently, she got a solid win rate of 52.1%.
Build:
With the recent buff to Guinsoo's Rageblade in Patch 14.3, Vayne's on-hit build is now way stronger. If you're playing her, start with Blade of The Ruined King, followed by Guinsoo's.
After that, choose between Terminus, Wit's End, or Guardian Angel based on what's happening in the game.
Powerspikes:
Talking about power spikes, Vayne becomes a beast late game. She deals percentage Max HP true damage, got high attack speed, and hardly has any cooldowns. A skilled Vayne player can carry the team in the later stages.
But early on, she might struggle a bit. With her short range, she doesn't do well against aggressive opponents. However, if she's got a good support, they can still dominate the lane.
Counters:
Tahm Kench is a pain for Vayne. Yes, she outranges him, but Tahm Kench is way too tanky for her to ever kill him. If he places his W right, there is also no way to tumble out of it, because the hitbox is too big. Most of the time, Tahm Kench's Q is also too fast to dodge for Vayne.
1. Yorick
And our number 1 in Patch 14.4 is Yorick! But with a more unusual build than you might think. Right now, he got an incredible win rate of 52.9%.
Build:
Instead of going a tanky or bruiser-like build, you should go for Profane Hydra first, into Seryldas's Grudge. Afterward, you can go for Opportunity, Edge of Night or Eclipse.
Powerspikes:
In the early game, you might struggle a lot. If your opponent punishes you and your ghouls correctly, there is not much you can do. However, later on in the game, Yorick starts to shine. Everyone fighting for Baron? Great! Free split push till you hit the Inhibitor for you!
Counters:
Zac struggles against Zed, Fiora and Yasuo. All of them can just dash out of your W. Fiora and Yasuo can also block your E, which is your only engage tool.

With changes on AP items, champs like Cassiopeia, and Teemo, yes Teemo,can really shine if they're played correctly.
While champs with a low win rate (less than 50%) might not look that great, they can also work really well as counter picks or in some niche situations. Gwen or Akshan are great examples.
In case you want to play Gwen or other AP champs, though, please keep in mind that tanks could buy the new MR item Kaenic Rookern, granting 80 MR and a magic shield on top of that, giving you a way harder time than before.

Yes, AD LeBlanc was a thing in top lane. But with tons of nerfs, you finally need to give up on her. You're not bobqinXD.
Briar and Shaco are just made for the jungle, and their kits really don't translate for top lane. You waste lots of their potential if you draft them for top lane.
Zed got horrible top lane match-ups. If you want to play him, please do it in the Mid Lane.
Last but not least, we got our problem child: Ryze. It's really hard to play him efficiently, so it would be more worth to play other AP top laners like Gwen. We'll probably need the 6th rework on him. I am not kidding, it would be the 6th if he'd receive one.
